Volunteer Policy AND Waiver (PLEASE READ THOROUGHLY):

1. It’s important for volunteers to follow procedures and act within the authorized scope of their duties.  Volunteers who activate without authorization, are considered to have been self-activated, and will be asked to resign from T-CEP.
2. I understand that as a NN captain/coordinator, I have the duty to call at least one other person in the event that a disaster affects Topanga and/or any time my neighborhood phone tree is activated.  I also agree that I will update the contact information annually for those whom I am responsible for calling.
3. I understand that if I use my own automobile in my volunteer service, I will keep in effect my auto liability insurance as required by California.
4. By submitting this form, I am signing up as a T-CEP volunteer and I authorize T-CEP to export my contact information into the outside company's Telephone Emergency Notification System (TENS).  T-CEP and the TENS company will not share or distribute your information gathered by this form and will use it solely for the purpose of providing emergency notifications.
5. I understand that, in the event of a disaster affecting Topanga, T-CEP will attempt to notify me in various ways.  I give T-CEP my express consent to contact me on any of the devices I register (phone, cell, text and email).
6. Neither T-CEP nor it’s officers, Board of Directors, volunteers, employees or affiliates, make any warrant, express or implied, or assume any legal liability or responsibility for the accuracy, completeness or usefulness of any information provided by submittal of this form.  I waive any claims that may arise against T-CEP, it’s officers, Board of Directors, volunteers, employees or affiliates, resulting from the information provided.  
7. While every attempt will be made to notify T-CEP volunteers of an emergency, there are factors that may impact the completion of those calls (e.g., loss of electrical power, telephone company line capacity, busy conditions, caller ID blocking, lack of time, etc.).  I understand that T-CEP, it’s officers, Board of Directors, volunteers, employees or affiliates, shall not be held liable, if for any reason, the TENS or it’s replacement fails to notify me before or during a disaster.
8. I understand that this service is intended only to assist me in my T-CEP volunteer duties and to complement my emergency plan.  If you believe you are in danger, you should not wait for or rely exclusively on T-CEP’s TENS, but immediately take all necessary precautions.  If you are directed to evacuate by emergency personnel, you should follow their directions.
